Ballad of a Thin Man inizia con un pianoforte oscuro e una voce che punta il dito. Bob Dylan la registrò il 2 agosto 1965 agli studi Columbia di New York, insieme ad altre tre canzoni dello stesso album: Just Like Tom Thumb's Blues, Queen Jane Approximately e Highway 61 Revisited. Dura quasi sei minuti, è prodotta da Bob Johnston, e finì per far parte di Highway 61 Revisited, il primo disco in cui Dylan usò una band di accompagnamento in tutte le tracce. Il testo ruota attorno a un certo Mr. Jones, un tipo che non capisce nulla di ciò che gli accade intorno. Dylan non chiarì mai chi fosse quel personaggio — disse che non lo avrebbe rivelato perché lo avrebbero querelato, e che l'unica cosa che confermava era che porta le bretelle. Quell'opacità è parte del gioco: lo stesso Dylan ammise che la gente disseziona le sue canzoni come fossero conigli senza mai capire di cosa parlino.
